MRAs for HFpEF / mildly reduced EF
Finerenone · spironolactone · eplerenone: different evidence and roles
Detailed source records support the one-page handout. Different trials, populations, follow-up periods and stopping definitions cannot be compared as a drug ranking.
HFpEF and HFmrEF agent roles
FINEARTS-HF and the current US finerenone HF label include EF ≥40%. This extends below usual HFpEF EF ≥50% into mildly reduced EF; it is not evidence restricted to EF ≥50%. The July 2026 ACC ECDP section 5.1.2 identifies finerenone as the preferred MRA, with spironolactone reasonable when cost or tolerance prohibits it. Eplerenone lacks demonstrated clinical outcome benefit in HFpEF, although the pathway allows consideration for spironolactone-related gynecomastia. Do not imply class-wide identical benefits or finerenone superiority in a direct comparison. [2] [1] [4] [3] [6]
TOPCAT enrolled EF ≥45% and its overall primary outcome was inconclusive. HF hospitalization alone was reduced, but regional variation, drug exposure/adherence findings and contemporary evidence limit confident extrapolation. The 2022 HFpEF Class 2b recommendation predates FINEARTS and the 2026 pathway. The 2026 ECDP discusses SPIRIT-HF as an abstract-only, underpowered trial with high stopping; no unsupported numerical efficacy claim is added here. [2] [1] [4] [3] [6]
Endpoints and denominator discipline
FINEARTS randomized 3003/2998, median follow-up 32 months. The primary analysis counts 1083/1283 total first and repeat HF events plus cardiovascular deaths, rate ratio 0.84 (95% CI 0.74–0.95). The same endpoint affected 624/719 distinct people (20.8%/24.0%). A crude 3.2-per-100 difference in people is not the recurrent-event effect estimate. Total worsening HF events were 842/1024 in 479/573 people. Cardiovascular deaths 242/260 yielded HR 0.93 (0.78–1.11), so do not promise longer survival from this trial alone. [2] [6] [7]
FINEARTS Table 3 safety N=2993 per arm covers treatment through three days after permanent stopping. Potassium >5.5 used N=2898/2889, events 413/199; >6 events 86/41. Measured systolic BP <100 used N=2911/2904, events 538/361; this is not the reported hypotension adverse reaction 226/2993 versus 140/2993. Canadian Table 5 also reports hyponatremia 57/2993 versus 27/2993. US label HF worsening-renal-function-related events were 18% versus 12%, including renal impairment 7%/4%, eGFR decreased 5%/4%, acute kidney injury 4%/2%, renal failure 3%/2%; overlapping grouped categories must not be summed. [2] [6] [7]
Stopping: non-death, adverse event and potassium subsets
FINEARTS non-death stopping 611/2993 (20.4%) versus 616/2993 (20.6%) is broader than permanent adverse-event stopping 3.2% versus 2.8%, and broader than permanent hyperkalemia stopping 0.4% versus 0.2%. The latter two percentages come from the HF-specific Canadian regulatory monograph, not reverse-calculated counts; its mean exposure approximately 25 months differs from the US label mean 2.3 years and the original paper median 32-month follow-up. These measures should retain their original attribution. The manufacturer HF site https://www.kerendiahcp.com/indication/heart-failure corroborates 3.2%/2.8%. [2] [7] [1]
The frequently quoted finerenone hyperkalemia stopping 1.7% versus 0.6% describes pooled CKD/T2D trials and is not FINEARTS. TOPCAT permanent all-reasons stopping was 590/1722 versus 541/1723; withdrawing from study participation 160/1722 versus 151/1723 was a different measure. A whole-trial TOPCAT AE-only or hyperkalemia-only permanent stopping figure was not verified from an accessible original appendix. No comparable verified real-world HFpEF/finerenone stopping estimate was identified; do not import the SwedeHF HFrEF estimate. [2] [7] [1]
Current US finerenone HF dosing and safety
Finerenone: do not initiate if K >5.0 mmol/L; initiation at eGFR <25 is not recommended. Start 10 mg daily at eGFR 25–<60, or 20 mg at ≥60. HF targets are 20 mg or 40 mg respectively based on eGFR at initiation; recheck K/eGFR four weeks after starting or adjusting and periodically. The HF potassium dose algorithm differs from the CKD/T2D algorithm. At K 5.5–<6, reduce or withhold according to current dose; at ≥6 withhold and follow the restart rules. Do not substitute a generic one-size-fits-all threshold for the label table. [6] [5]
Strong CYP3A inhibitors, adrenal insufficiency and hypersensitivity are contraindications. Avoid grapefruit and strong/moderate CYP3A inducers; moderate/weak inhibitors require monitoring rather than automatic categorization as absolute contraindications. Pregnancy data are limited with animal developmental toxicity; discuss pregnancy. Breastfeeding should be avoided during treatment and for one day afterward. Review all potassium-raising medicines and renal risks. Spironolactone monitoring remains within one week of start/titration. [6] [5]
Price verification and insurance boundaries
All price checks are dated September 5, 2026. GoodRx spironolactone 25 mg, 30 tablets was the standard $9.73 Walgreens coupon for the page default ZIP 85001; its panel says updated August 11, 2026. One tablet daily makes this 30 days. Paid Companion and differently specified teaser prices were excluded. No fully specified accessible eplerenone or finerenone GoodRx quote was verified. [8] [9] [10] [11] [12] [13] [14] [15] [16]
Blueberry public UI: 30 tablets at one daily, nonmember spironolactone 25 mg NDC 59746-0216-09 $11.87 and eplerenone 25 mg NDC 69367-0307-09 $15.69. These include acquisition plus 8% markup, $1.15 supply cost and $10 professional fee. Add $3.99 shipping to obtain $15.86 and $19.68. No verified finerenone result does not establish that the pharmacy cannot supply it. Confirm service area, SKU, tax, current quote and delivery. [8] [9] [10] [11] [12] [13] [14] [15] [16]
KFF 2025 tier copays describe covered workers in employer plans with at least three tiers and copayments; they do not estimate any specific MRA copay. Medicare 2026 standard benefit has a maximum $615 deductible, 25% initial coinsurance and $2,100 annual qualifying out-of-pocket threshold. A plan-negotiated covered $40 fill would imply $10 standard coinsurance only after the deductible and before the cap; this is arithmetic, not a verified MRA price. Plans can differ. Extra Help eligibility and generic/brand caps vary with circumstances; some pay less. Cash coupons used outside Part D do not count toward its deductible or cap. [8] [9] [10] [11] [12] [13] [14] [15] [16]
Finerenone commercial assistance
Bayer advertises as little as $0 per month for eligible commercially insured adults, with maximum annual savings of $3,000. Government-insured patients are excluded and approved-indication and program rules apply. This is an assistance offer, not a cash retail price or typical copay. No enrollment or coverage check was performed. [17]
